Grandparents could receive up to £4,800 a year for providing childcare if the Conservatives run the Welsh Government after May.
The Welsh Tories said it would recognise the "vital role" grandparents play in raising children.
The payments would amount to £2,400 a year per child, capped at two children per household, meaning grandparents looking after two or more children could receive up to £4,800 a year.
Parents would be able to nominate one grandparent per family, who provides care for children aged nine months to four years old.
Darren Millar, leader of the Welsh Conservatives, said the offer "empowers parents" to make the "best choice" for their families.
He said: "At this election, I am committed to putting forward the most pro-family policy offer Wales has ever seen.
"Our Welsh Conservative childcare offer recognises the vital role that grandparents play in delivering care for their grandchildren, extends access to more free childcare, and empowers parents to make the best choice for them and their family.
"These plans are affordable, deliverable, and will help to get Wales working."
Currently, parents of three and four-year-olds in Wales can claim up to 30 hours of free childcare a week, provided they meet certain criteria.
The Conservatives are also seeking to extend this offer to working parents of children aged nine months to four years old.
