Rail investigation uncovers signaller error in Herefordshire accident

The Rail Accident Investigation Branch (RAIB) has published a report on the collision between a passenger train and an agricultural trailer at Nordan Farm user worked crossing, near Leominster, Herefordshire.

On 22nd May 2025, a train travelling at approximately 80 mph between Ludlow and Leominster collided with the trailer, resulting in minor injuries to six passengers and one member of staff.

The report found the accident occurred because the signaller "gave permission to the tractor’s driver to use the crossing, despite the proximity of the approaching train."

The signaller did not check signal box equipment indicating the train's presence, leading to the oversight.

RAIB attributed the signaller's error to "an interruption to their established routine," along with factors such as workload increase, distraction, and fatigue.

Further investigation found Network Rail's risk assessment process for level crossings failed to recognise increased seasonal use risks, describing this as "an underlying factor."

Additionally, RAIB noted Network Rail's lack of a clear process for deciding vehicle classifications at crossings.

Two recommendations have been made for Network Rail.

The first calls for improved risk management processes at crossings with high variations in use.

The second advises reviewing previous responses to RAIB recommendations related to vehicle classifications at crossings.

The RAIB also identified four learning points, including reminders for staff to comply with mobile phone policies and assess fatigue levels.

In response to the report, a spokesperson for Network Rail said,

“Safety remains our highest priority for both passengers and our colleagues, and we have fully supported the independent investigation undertaken by the Rail Accident Investigation Branch into the incident at Nordan Farm user worked crossing last year. Our thoughts remain with everyone affected by this incident.

“While incidents of this nature are rare, it’s vital we understand exactly what happened and where we can do better. We are carefully considering the report’s findings and recommendations, and work is already underway to review our processes and guidance, refresh training where needed and make sure the lessons from this incident are shared across the railway.

“We are committed to learning from this incident and continuing to improve safety across the Wales and Borders railway and the wider rail network.”
