Warning issued over future risks of drug-related deaths in HMP Peterborough

Concerns raised following the investigation into Fallon Adams' death in prison

Concerns about potential future drug-related deaths in HMP Peterborough have been raised following an investigation into the death of 37-year-old Fallon Adams.

A Prevention of Future Deaths Report (Regulation 28), filed by area coroner Simon Milburn, revealed issues surrounding Adams’ death, which occurred between 8th and 9th February 2023.

Adams, an inmate at HMP Peterborough for eight days before her passing, died as a result of mixed drug intoxication, according to the inquest's conclusion on 29th November 2025.

The report highlights that she was prescribed sedative medications including methadone and chlordiazepoxide for withdrawal treatment. Adams reportedly also consumed illicit substances obtained within the prison.

The coroner noted that no clear warning or advice had been provided to Adams about the risks of combining prescribed medications with non-prescribed substances, nor was it evident that changes were being implemented to address this risk.

The report also pointed out the prison staff’s failure to carry out adequate welfare checks, with observations of respiratory movement and general well-being described as unsatisfactory.

The Northamptonshire Healthcare NHS Foundation Trust is required to respond to the coroner's report by 23rd February 2026, outlining actions to address these concerns or explaining why no action is proposed.

Copies of the report have been sent to Adams’ family, Sodexo, and the Chief Coroner, among others. Future publication of either the report or responses will be considered by the Chief Coroner.
