Worcestershire hospital's updated feature allows for more 'joined-up approach' for care of stroke patients
A set of new patient information boards have been rolled out on the stroke unit at Evesham Community Hospital
Work has been carried out by a Worcestershire hospital in a bid to make it easier for staff to share and fully understand the important information of a patient.
New patient information boards have been added to the stroke unit at Evesham Community Hospital so details for that person, such as their rehabilitation goals, transfer techniques, mobility, and communication needs, are clear for the clinical team looking after them.
Created in partnership with therapy staff and Herefordshire and Worcestershire Health and Care NHS Trust’s patient experience, participation and engagement team, the Trust said it has been "warmly welcomed" by staff, patients and families.
“Stroke rehabilitation often involves complex and highly individualised care," said Jonathan Hyam, a specialist occupational therapist at Worcestershire stroke rehabilitation centre.
"The new boards support a more joined-up approach, helping nursing, therapy, medical and support staff to work together more effectively and support patients in the best way they can.”
The Trust said the boards represent a "simple but effective approach" that is already making a "positive difference to stroke rehabilitation".
They've also added a QR code on them which can link patients and their families directly to Care Opinion, where they can give overall feedback on their time spent there.
Director of patient experience, digital and innovation, Gemma Artz, said this move makes it really easy for people to share their experiences of Trust services in their own words.
“It’s important people know they can tell their story and be heard," she said.
"This kind of feedback helps us understand what’s working well and where we can improve, so we can keep building on the good work already happening at Evesham Community Hospital.”
