Latest plans to create 27 flats at the site are under consideration by Leeds City Council.
The 49-bed home on Fieldhouse Walk closed in 2019 after concerns were raised by the Care Quality Commission.
Developer Lacuna Property Development is seeking to redevelop the site after a previous scheme for 14 flats did not go ahead.
Photographs show how the property’s condition worsened in between building surveys carried out in 2022 and 2025.
The latest show boarded up windows and doors on the building’s exterior and crumbling plasterwork inside.
A planning report said: “The internal photographs further evidence the advanced state of deterioration within the building.
“Widespread water ingress has resulted in significant plaster failure, staining and detachment of decorative finishes.”
Rotting timber, cracked ceilings and damp seeping into the Victorian building were also among the concerns.
The report by HG2 Architects said: “The images demonstrate that the building is not merely vacant but actively deteriorating.
“The absence of occupation and maintenance has accelerated fabric decay, reinforcing the urgency of securing a viable and comprehensive scheme of repair and reuse.”
